Sin,?With reference to the article 'Typhus in the Northern Circars' that appeared in the l.M G. for September 1949 (page 393) I may please be allowed to put forth my views about it. While one agrees that some, if not many, of the pyrexias of unknown origin may be typhus-like or typhus fevers, it is not very safe to diasnose a case of typhus clinically especially when it relates to a couple or stray cases. In an epidemic, a case of clinical tynhus, even with a negative Weil-Felix, would easily fit in. The same may be said or an endemic area. Iam of the opinion that a rise in titre should be taken as a diagnostic significance rather than depending upon

We screened Orientia tsutsugamushi from 385 domestic rodents and 19 humans with scrub typhus in rural Tai'an District, Shandong Province, a new scrub typhus epidemic area in northern China. Sequence analysis identified 7 genotypes in the rodents, of which 2 were also identified in the humans.

Azithromycin was given to mice and humans infected with strains of Orientia tsutsugamushi from northern Thailand, where drug-resistant scrub typhus occurs. Azithromycin and doxycycline yielded comparable mouse survival rates (73 and 79%, respectively; P > 0.5). Symptoms, signs, and fever in two pregnant women abated rapidly with azithromycin. Prospective human trials are needed.
